The evidence pyramid, in dental context#

Expert opinion and case reports#

Useful for generating hypotheses or flagging rare complications, but the lowest level of evidence — a single case does not establish general effectiveness.

Case-control and cohort studies#

Observational studies that can show associations, such as a link between periodontal disease and a systemic condition, but cannot prove causation on their own.

Randomized controlled trials#

The standard for testing whether an intervention causes an outcome, assuming the trial is well designed, adequately powered, and has reasonable follow-up.

Systematic reviews and meta-analyses#

These sit at the top of the pyramid because they synthesize multiple studies, though their quality still depends entirely on the quality of the studies included.

Why the hierarchy matters in daily practice#

A single study reported in a manufacturer's marketing email should carry far less weight in your decision-making than a systematic review published in a peer-reviewed journal — even if the marketing email is easier to read.

The check is fast. Ask what design produced the finding, how many patients it covered, and whether anyone independent has reproduced it. Those three answers usually tell you whether a result is worth acting on today or worth watching for another year.
